Proof of Current Health & Immunizations
- We are required by state law to have a current physical and immunization record for each child
- We must have documentation on file before children can attend the center
- Vaccination records must be kept up-to-date in order for your child to remain enrolled
- Please bring us a copy of your child’s newest immunizations after each doctor visit, we will send home a reminder when it is time to update
Medicine Administration Policy
- Child can receive prescription medication in the original container with the child’s name, date, dosage, and dosage intervals
- Sample medications may only be given with an accompanying physician’s note with dosage, dosage intervals, amount, date, and signature
- All parents must complete and sign the medication permission form
